Wherever home is, our care comes to you.

United Hospice has no building of its own. Our team travels to wherever the person you love actually lives, across ten Illinois counties.

Home means whatever it means to you.

A house or apartment

The most common setting, and the one most people ask for. We bring the care, the equipment and the medication to the door.

A family member's home

A daughter's spare room, a son's downstairs. If that is where they are, that is where we come.

A nursing home

We care for patients who live in nursing homes across our counties, working alongside the staff already there.

Assisted living

The same, for residents of assisted living communities. Our team visits them where they live.

A family sitting together in their living room
An older woman in a wheelchair being wheeled slowly through a sunlit room

To be clear about one thing: we serve patients who live in nursing and assisted living communities. We do not run one. Nobody is ever asked to come to us.

Ten Illinois counties.

Cook · DuPage · Lake · McHenry · Will · Kankakee · Kendall · DeKalb · Winnebago · Boone

Not sure whether you are inside the area? Call and ask. If we cannot reach you, we will point you to a hospice that can.

ILLINOIS

Our office.

1420 Renaissance Drive, Park Ridge, IL 60068

This is our administrative office. It is where the paperwork lives, not where the care happens, and there is nothing there for a patient or a family to visit. Everything that matters happens where the patient is.

Someone is reachable, always.

On-call nurses are available twenty-four hours a day, seven days a week, including evenings, weekends and holidays. That is availability, not office hours. When you call at night, a hospice nurse answers.

Tell us where they are. We'll come to them.

Ten counties, and a nurse on call every hour of the year.
