You keep your role as attending physician, you approve the plan of care, and you receive updates as the picture changes.

Eligibility at a glance.

Two physicians certify a prognosis of six months or less should the illness follow its expected course. Beyond that, the clinical indicators we see most often:

  • Declining functional status — PPS or Karnofsky at or below 70 percent
  • Unintentional weight loss, low albumin, or declining oral intake
  • Recurrent hospitalizations or emergency department visits
  • Dyspnea at rest, or increasing oxygen requirement
  • Recurrent infections, including aspiration pneumonia and urinary tract infections
  • Stage 3 or 4 pressure ulcers
  • Increasing dependence in activities of daily living
  • Treatment burden outweighing benefit, or a patient declining further curative treatment

If you are unsure whether a patient qualifies, call. We will review it with you, and we will tell you honestly if it is too early.
