United Hospice is a Chicago hospice provider dedicated to comfort, dignity and trust at the end of life.
Who we are.
United Hospice provides expert, compassionate hospice care for the elderly and those in end-of-life stages, at home or wherever home happens to be. Our team of nurses, physicians, social workers and aides travels to patients across ten Illinois counties, easing pain and symptoms while supporting the whole family, emotionally and practically.
We are here for families making one of the hardest decisions of their lives, and for the physicians who trust us with their patients.

We know how hard the end of life is, physically and emotionally. It is your life, and your decisions matter. Our team works with you and the people who love you to build a plan of care that reflects what you need, what you want, and what you expect. We are here to ease discomfort, bring calm, and support a peaceful transition.

How we work.
- One interdisciplinary team, coordinated around one person and one family
- A plan of care written for this patient, approved by their own physician
- A nurse reachable twenty-four hours a day, every day of the year
- Care delivered wherever the patient lives, never at a location of ours
- Support that continues for the family after a death, for as long as it is wanted
- Careful screening and training of everyone who enters a patient's home
